Location: Hays, KS (travel to outreach clinics– center vehicle provided)
Pay: $23.17 per hour
Job Type: Full-time, In-Person
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
We are excited to announce we are now hiring a Primary Care Liaison at High Plains Mental Health Center! As the Primary Care Liaison you'll build lasting relationships with primary care providers, hospitals, and community partners. Your role is key in making sure patients are guided smoothly and confidently into the services they need.

- Serve as the primary point of contact between HPMHC and local primary care clinics, hospitals, and community agencies.
- Coordinate referrals and transitions to mental health services—ensuring timely, smooth care for patients across rural communities.
- Educate patients, families, and providers about the referral process, mental health services, and community resources.
- Manage clinic schedules and support the delivery of telehealth psychiatric services through the TBHN Expansion Grant.
- Build strong, trusting relationships with medical and behavioral health professionals, helping expand access to mental health care.
- Maintain accurate documentation and uphold HPMHC's high standards for communication, compliance, and care quality.
- Travel to outreach clinics to support underserved areas—fueling a statewide effort to improve mental health access.
- Current Kansas LPN license in good standing.
- At least 1 year of nursing experience, ideally in mental health or a physician’s office setting.
- A people person—someone who thrives in conversation, builds trust quickly, and loves to educate and empower others.
- Passion for care coordination and improving mental health outcomes in underserved areas.
- Organized, proactive, and skilled in handling multiple priorities.
- Comfortable with technology and healthcare communication systems.
- Valid driver’s license and insurable driving record (travel required; center vehicle required).
